The ideal candidate will have experience in a Hospice capacity. This role involves competently assessing, coordinating, and evaluating the spiritual needs of hospice patients and families, and providing direct spiritual care in coordination with the patient care team. The Chaplain develops, documents, and delivers a hospice spiritual care plan. Key responsibilities include fostering and maintaining collaborative relationships with patients, their family/support group, physicians, and other healthcare providers through timely and effective communications and attendance at meetings and patient/family care conferences. The Chaplain must adhere to policies, industry standards, best practices, and applicable laws/hospice/Conditions of Participation, regulations, and codes to promote a quality, highly reliable patient experience. Additionally, the position requires providing worship services, rituals, memorials, and support groups for hospice patients and families in the community or wherever they call home, and for staff as requested.
