About The Position

The Hospice Chaplain at Adoration Home Health and Hospice plays a vital role in providing spiritual care services to patients and their families. This position involves coordinating with an interdisciplinary team to ensure an optimal care experience for patients in hospice care. The Chaplain is responsible for conducting spiritual assessments, developing individualized care plans, and offering support during difficult times, making a meaningful impact in the lives of those they serve.

Requirements

• Graduate of an accredited theology or chaplaincy program (BD, MDiv or equivalent theological degree)
• Experience working with death and dying, hospice experience preferred
• Comfort in an interfaith setting
• Ability to accept different lifestyles, cultures, beliefs and values
• Knowledge of and commitment to hospice philosophy of care

Responsibilities

• Assure spiritual assessment of patients and families/caregivers in the hospice program and provide direct spiritual care as appropriate
• Participate in the development of an individualized plan of care to meet spiritual needs
• Serve as a liaison to community chaplains and spiritual counselors
• Maintain records and documentation of services provided
• Attend patient care conference and interdisciplinary group meetings
• Provide funeral or memorial services for patients as requested
• Ensure compliance with all state and federal legal and regulatory requirements
• Additional responsibilities as assigned
