About The Position
The CFSP Foster Care/Adoption Care Manager (Managed Care Coordinator) is a field-based role that primarily operates in the field, traveling to client sites or designated locations as required. This position is part of a partnership with North Carolina DHHS to implement a statewide Medicaid Plan aimed at supporting Medicaid-enrolled infants, children, youth, young adults, and families in the child welfare system. The role involves managing the member’s individual service plan within the NC CFSP Program, ensuring access to integrated health care services including physical health, behavioral health, and long-term services and supports. The Care Manager will provide integrated whole-person care management, collaborate with multidisciplinary teams, and conduct assessments to identify and manage member needs.
