About The Position
The CFSP Foster Care/Adoption Care Manager (Managed Care Coordinator) is a field-based role that primarily operates in the field, traveling to client sites or designated locations as required, with occasional office attendance for meetings or training. This role is part of a partnership with North Carolina DHHS to operationalize a statewide Medicaid Plan designed to support Medicaid-enrolled infants, children, youth, young adults, and families served by the child welfare system. The position is responsible for the overall management of the member’s individual service plan within the scope of the NC CFSP Program, ensuring that members receive seamless, integrated, and coordinated health care. The role involves providing integrated whole-person Care Management, collaborating with multidisciplinary care teams, and conducting assessments to identify and manage member program needs.
