About The Position

The Coordinated Family Supports (CFS) Specialist is responsible for providing comprehensive case management (small caseload) and support services to adults with intellectual and developmental disabilities who choose to remain in the family home. This role involves coordinating access to resources, facilitating service delivery, and supporting long-term planning to promote stability and independence. The CFS Specialist collaborates with families, service providers, and community agencies to ensure individualized, culturally responsive support across Contra Costa and potentially Alameda Counties.

Requirements

  • 2-4 Year Degree preferably in the human services field; OR General studies with a minimum of 1 years of experience in developmental disabilities or related field; OR A reasonable combination of the above education and experience requirements.
  • Valid California driver’s license
  • Clean driving record (DMV printout is required)
  • Current vehicle registration and use of a vehicle in good running condition during work hours.
  • Must have valid auto insurance with $100,000-$300,000 bodily injury liability insurance
  • Must be able to pass DOJ fingerprinting/criminal background check
  • Must be able to obtain CPR/First Aid certification
  • Willingness and ability to perform essential functions such as answering phone and computer-based tasks.
  • Ability to work collaboratively with consumers and co-workers.
  • Ability to work independently and handle multiple priorities.
  • Strong verbal and written communication skills.
  • Tuberculosis Clearance Screen.
  • Provide documents required for I‐9 employment eligibility.
  • Understanding of developmental disabilities, mental health, and adult protective services.
  • Strong organizational, record-keeping, and analytical skills.
  • Proficient in conflict resolution and community navigation.
  • Effective interpersonal and team communication.

Responsibilities

  • Identify and coordinate supports for individuals with I/DD to remain in the family home.
  • Conduct intake, assessment, and service planning.
  • Assist with navigation of public services (e.g., Medi-Cal, IHSS, Social Security).
  • Facilitate education related to diagnosis and needs.
  • Coordinate consistency and quality of services from support professionals.
  • Assist with appointment scheduling and coordination.
  • Identify transportation and backup services as needed.
  • Support long-term planning including for those with aging caregivers.
  • Deliver training to promote independence and inclusion.
  • Ensure services are culturally and linguistically appropriate.
  • Prepare and submit quarterly service progress reports.
  • Report Special Incidents and Suspected Abuse as required.
  • Report abuse within 24 hours and notify supervisors of significant client changes.
  • Perform general administrative tasks as assigned.
  • Maintain Thrive’s stability and reputation through professional conduct.
  • Follow all applicable policies, procedures, and laws.
  • Practice strong communication and support team efforts.
