About The Position

Bost Inc. is seeking a full time Community Employment Supports (CES) Waiver Manager in Fort Smith, Arkansas. Applicant must have BS/BA degree or acceptable experience in Human Services or related field or a minimum of one year’s experience in Human Services or related field. Applicant must also have a valid driver’s license and reliable transportation with at least liability insurance. As a CES Waiver Manager you will be the liaison between individual and assigned Provider Led Arkansas Shared Savings Entity (PASSE) to ensure the delivery of services meet the needs, goals, and objectives identified in the Person Centered Service Plan (PCSP). Some responsibilities will include implementing PCSP plans for individuals on assigned caseload, ensure budget information is completed and submitted timely and ensure the individual attends all medical appointments and obtain required paperwork. Your will also be responsible to make monthly contact with each consumer on assigned caseload, complete the home requirement checklist as needed, ensure all staffing needs are met, and that the consumers plan of care is fully utilized. Other duties are assigned as needed. Requirements

  • BS/BA Degree or acceptable experience in Human Services or related field or minimum of (1) one years’ experience in Human Services or related field.
  • Valid driver’s license and reliable transportation with at least liability insurance.
  • Pass a background check, reference checks, and drug screening.

Responsibilities
