The Certified Registered Nurse Practitioner (CRNP) will provide comprehensive and compassionate care across the lifespan, with a focus on patients living with or at risk for HIV. The ideal candidate will work collaboratively with other medical and mental health providers, nurses, and support staff to deliver high-quality, compassionate patient-centered care.   • Deliver primary and specialized care for patients of all genders living with HIV, including diagnosis, ART initiation, monitoring, and adherence counseling
  • Provide pre-exposure prophylaxis (PrEP) and post-exposure prophylaxis (PEP) as appropriate, STI screenings, and risk-reduction counseling
  • Coordinate care and referrals for behavioral health, case management, housing, substance use services, and other social determinants of health
  • Advocate for patient-centered care using trauma-informed, culturally competent approaches
  • Monitor and manage co-occurring conditions, including hepatitis, and chronic diseases
  • Perform and interpret diagnostic tests such as Pap smears, STD screenings, complete blood counts (CBCs), electrocardiograms (EKGs) and radiographs (x-rays)
  • Address gynecologic and reproductive health needs including contraception, prenatal/postnatal support, menopause management, and cervical cancer screening as needed
  • Educate patients on health promotion, disease prevention, and reproductive health
  • Recommend interventions to modify behavior associated with health risks
  • Collaborate with interdisciplinary teams for continuity of care
  • Consult with or refer patients to appropriate specialists when conditions exceed the scope of practice or expertise
  • Develop treatment plans based on scientific rationale, standards of care, and professional practice guidelines
  • Perform primary care procedures such as suturing, splinting, administering immunizations, taking cultures, and debriding wounds as needed and appropriate
  • Perform routine or annual physical examinations
  • Prescribe medications based on efficacy, safety, and cost as legally authorized.
  • Accurately document patient care using electronic medical records (EMR) in a timely manner
  • Participate in quality improvement initiatives and continuing education.
  • Keep abreast of regulatory processes and payer systems such as Medicare, Medicaid, managed care, and private sources
  • Maintain departmental policies and procedures in areas such as safety and infection control
  • Greet and acknowledge patients according to agency standards
  • Receive, record and relay messages accurately, completely and legibly
  • Speak with others using clear, professional and polite language
  • Develop and maintain positive working relationships with others, support the ACG team to reach common goal
  • Coordinate and support the medical support staff by leading by example

  • After a conditional offer of employment has been signed, the applicant is subject to a criminal background check, drug test, and employment verification.
  • Active CRNP license in the State of Pennsylvania
  • 5+ years of experience as a CRNP
  • Experience providing direct HIV prevention and treatment care
  • Experience engaging with African diaspora communities in service delivery
  • Experience working in underserved or high-risk communities
  • Availability to work one Saturday/ month
  • Familiarity with trauma-informed care practices
  • Must be able to comfortably work with people of all lifestyles
  • Knowledge of addictions and their association with HIV disease

  • Bilingual (Spanish/English) is a plus
