About The Position

The Certified Recovery Specialist is a member of the care team providing services to a diverse population seeking or receiving outpatient services for substance use disorders (SUD) and complex medical and behavioral health conditions with recent interface with the acute care system at Jefferson. The CRS improves the delivery of outpatient services by providing interventions including but not limited to providing recovery education; supporting patients in crisis by identifying acute stressors and attempting to mediate them; advocating for patients to ensure their needs are adequately acknowledged and addressed; supporting families who present to the Bridge Clinic requesting information about substance use disorders and co-morbid mental disorders. Support access to and retention in care by proactively communicating with patients by telephone and in community-based settings. Provide recovery education for every phase of the recovery journey from pre-recovery engagement, recovery initiation, recovery stabilization, and sustained recovery maintenance. Provide a model for both people in recovery and staff by demonstrating that recovery is possible. Actively identify and support linkages to community resources (communities of recovery, MOUD, educational, vocational, social, cultural, spiritual resources, mutual self-help groups, professional services, etc.) that support the recovering persons goals and interests. Provide vision-driven hope and encouragement for opportunities at varying levels of involvement in community-based activities (e.g., work, school, relationships, physical activity, self-directed hobbies, etc.). Develop relationships with community groups/agencies in partnership with others in Jefferson Health. Identify barriers (internal and external) to full participation in Jefferson Health or community resources and develop strategies to overcome those barriers. Maintain contact either in-person, by phone and/or e-mail with recovering person for 6 months once they engage in a recovery program to ensure ongoing success and to provide re-engagement support in partnership with others in the Jefferson Health if needed. Serve as an essential link between patients and all other care providers. The CRS is responsible for interacting with patient service representatives to expedite scheduling for follow-up and acute care appointments within Jefferson Health. Meet with patients at least monthly to assess compliance, address patient needs and gain insight into unmet needs. Coordinate information and patient needs (social service, health, and medication) with appropriate interdisciplinary provider: PCP, social work, MOUD provider, D&A provider, etc. Follow patients throughout the care continuum, including inpatient admissions, discharges from all levels of care, and collaborate with inpatient care management resources. Track all data requirements related to client activity and requirements set within the Bridge Clinic, Behavioral Health or other Jefferson Health key departments.
