About The Position

The Certified Recovery Mentor plays a crucial role in providing comprehensive case management services aimed at supporting clients in their recovery journey. This position involves a combination of referral linkage, individual counseling, family support, group counseling, and case management services to ensure that clients engage successfully in treatment and that their families are equipped to support this ongoing involvement. The mentor may work in either a residential or outpatient setting, depending on the needs of the clients and the services provided by LifeWorks NW. Located at LifeWorks NW's Rockwood site, the Certified Recovery Mentor is responsible for screening and selecting clients for the mentoring program. They develop individualized service plans for each client, which include a service reporting plan for all referrals. Within the first five working days following a client's intake into services, the Recovery Mentor collaborates with the client and, when appropriate, their family members to create a detailed case management plan. This plan is essential for guiding the client's recovery process and ensuring that they receive the necessary support. The Recovery Mentor maintains daily contact with clients for up to the first five weeks post-release, assisting them with various issues such as recovery support, housing, employment, family adjustments, legal matters, medical needs, and benefits. As the client progresses, the frequency of contact is gradually reduced to twice monthly or monthly by the ninetieth day after release. The mentor is also responsible for helping families implement the case management plan and maintaining regular communication with the client's primary therapist and other service providers. Documentation is a critical aspect of this role, as the Recovery Mentor must document and maintain client records in an electronic healthcare system within 24 hours of meetings to ensure compliance with agency guidelines and confidentiality standards. Regular meetings with staff from social service agencies are also required to update them on LifeWorks NW services, identify unmet needs, and facilitate referrals. The position requires attendance at various informational and clinical meetings, including all-agency meetings, team meetings, and intra-agency trainings. All services provided must be culturally appropriate, and the mentor must meet documentation and work accuracy standards as defined in the Performance Evaluation. The role may also involve travel between sites or in the community, including driving clients when necessary.
