How You’ll Impact Patient Care: Your responsibilities may include, but are not limited to: Interviewing patients or caregivers to obtain detailed medication information. Reviewing prescriptions, over-the-counter drugs, supplements, and herbal products for accuracy. Reconciling medication lists to identify discrepancies or potential safety concerns. Communicating with pharmacies, hospitals, group homes, and nursing facilities etc. to verify doses and frequencies of medications and last filled dates. Collaborating with pharmacists, nurses, and physicians to ensure safe and effective medication use. Maintaining confidentiality and complying with all regulatory and institutional guidelines. Works well with others and organizes daily tasks for maximum efficiency. This role requires attention to detail, strong communication skills, and a commitment to patient safety, as accurate medication lists are critical for preventing errors and ensuring optimal care.
