About The Position

High Point provides a range of community-based services including Community Based Recovery Support (CBRS). CBRS was created to provide additional support to people as they work on their recovery within their communities. Often, clients who have been in an addiction treatment or Section 35 facility and are transitioning back into their communities' benefit from extra support. High Point & Affiliated Organizations is a health and human service agency whose mission is to treat and prevent substance use disorders and mental illness. High Point has programs located throughout Southeastern Massachusetts offering a full continuum of care for substance use and mental health treatment, including inpatient, outpatient, residential, and community-based services. Programs and services also assist survivors of abuse, violence, and families experiencing homelessness. High Point believes that everyone has inherent goodness, worth, and dignity. Requirements

  • Must present as courteous and professional at all times
  • Demonstrated flexibility and ability to perform multiple tasks
  • Must have excellent communication skills, including the ability to organize ideas in logical and clear fashion
  • Knowledge of basic computer skills and software programs including the ability to learn, use and train HPAO Electronic Health Record (EHR)
  • Must have the ability to work with others in cooperative and collaborative manner
  • An understanding of the relationship between substance use and mental health issues and/or self-help programs
  • Understand and maintain therapeutic boundaries and reinforce rules in a respectful manner
  • Comply with Federal Regulations, 42 CFR Part 2, Confidentiality of Substance Use Disorder and HIPAA Regulations
  • TB/Flu and Covid19 Assessment
  • Completion of the HPAO approved de-escalation program
  • Successful completion of all trainings upon hire and annually as required
  • High School Diploma or GED, preferred
  • One-year experience in substance use treatment and/or mental health field
  • Personal lived experience in mental health and/or substance use recovery (RC/CPS required)
  • Experience working with families with children with SMI (FSN, Family Partner)

Nice To Haves

  • Associates Degree in human services (optional)
  • CPR Certification, preferred

Responsibilities

  • Maintain assigned caseload
  • Providing direct care to residents – including assessments, individual counseling, discharge and aftercare planning, treatment plans and updates, weekly progress notes, daily groups facilitation, group note completion, and completion of bio-psychosocial assessments
  • Making referrals when appropriate
  • Responsible for each assigned residents’ medical records. All records are to be professional, complete and thorough with entries made in a timely manner
  • Responsible for linking resident’s with aftercare services prior to discharge and aiding the resident and his/her family access services when appropriate
  • Strengthen current and create new partnerships within the community to ensure that the clients develop the skills needed to sustain sobriety and/or housing
  • Review patients/family rights and responsibilities, emergency safety plans and grievance policy Communicating/involving family in treatment at patient/client/resident’s request
  • Maintain professionalism when connecting with individual’s collaterals
  • Attend and participate in all unit, facility meetings, case conferences, Treatment Team meetings, and mandatory trainings, as directed by the Clinical Supervisor /Director
  • Responsible for continued education in the field respective to their service level
  • Engagement with patients in community settings and inpatient units to offer services
  • Completion of initiation for services and all required documentation based upon services agreed upon
  • Ongoing monitoring and compliance with required documentation for services. Monitoring service authorizations and providing updated information for continued stay reviews
  • Care coordination activities to ensure referrals for services are completed
  • Monitoring consents & releases are completed upon admission and as needed thereafter. Assist with scheduling appointments for follow up with designated community providers
  • Participation in program initiatives, quality projects, and program support services
  • Engagement in training opportunities to enhance knowledge of mental health and SUD services
  • Engagement and attendance at community events with stakeholders, agencies and service providers
  • Ability to engage patients with their individualized recovery plan
  • Support community based needs including: appointments, attending self-help meetings, accessing community resources (ie: housing, SNAP/WIC)
  • Certified Peer Specialist: Application and acceptance to KIVA training- ability to complete training and certification within 12 months of hire
  • All other duties as assigned by Program Director / Clinical Supervisor
