About The Position

Community support staff focused on the Emerging Adult and Adolescent populations. As a part of the System of Care grant from DBHDD will provide services that consist of rehabilitative skills building, the development of environmental supports and resources coordination considered essential to assist a person in improving functioning, gaining access to necessary services in creating environments that promote recovery and support the emotional and functional improvement of the individual. The CPS-Y will also participate in the engagement and coordination of care of children, adolescents, and young adults with other systems and internal providers.

Requirements

  • Certified Peer Specialist-Youth in Georgia.
  • Must have a current driver's license.

Nice To Haves

  • Bachelor's of Social Work or Bachelor's degree in any behavioral science from an accredited college or university.

Responsibilities

  • Serve as the primary coordinator of behavioral health services and provide linkage to the community.
  • Facilitate access to general entitlements and psychiatric, substance use/abuse, medical services, crisis prevention and intervention services.
  • Maintain weekly contact with individuals on caseload.
  • Meet with consumers face to face a minimum of twice a month, ensuring at least 50% of contacts are face to face and 80% are outside of the clinic.
