About The Position

Requirements

  • High school diploma or GED or equivalent.
  • Minimum of two years of experience in the guiding principles of recovery that assist individuals to improve their health and wellness, live a self-directed life, and reach their full potential.
  • Valid driver's license per vendor agency requirements.
  • Active Certified Peer Recovery Specialist certification through the State of New Jersey.

Responsibilities

  • Respond promptly to all client needs in the hospital or emergency room setting.
  • Serve as a role model by sharing lived experience, knowledge, hope, and skills to encourage and promote recovery.
  • Educate individuals on all pathways to recovery, treatment options, and available support systems.
  • Link clients to community resources, including treatment programs, social services, recovery supports, and any other services to build and maintain recovery capital.
  • Build rapport, sustain, and maintain positive relationships with all clients, healthcare staff, and community partners.
  • Assist clients with gaining, maintaining, and developing skills and tools needed to sustain recovery and achieve personal goals.
  • Actively coordinate and support clients during their treatment engagement.
  • Guide clients in identifying relapse triggers and coping strategies to maintain recovery.
  • Document all client interactions, recovery plans, and progress notes in the designated electronic health record system.
  • Handle sensitive situations with confidentiality, professionalism, and respect.
  • Offer consistent follow-up to ensure continuity of care and support progress toward recovery goals.
  • Work closely with navigators and other team members to maintain continuity of care and create comprehensive recovery plans.
  • Participate actively in regular both individual and group supervisions, case reviews and training to enhance service delivery.
  • Comply with all program policies, funding requirements, and documentation timelines as outlined in the Lifeline Policy & Procedure manual.
