Summa Homecare & Hospice operates Mon-Fri, 8:30 am- 5 pm, covering territories including Brunswick, Hinkley, Wadsworth, Seville, Northfield Center, Richfield, Ghent, Peninsula, Twinsburg, Aurora, Hudson, and Streetsboro. As an Occupational Therapy Assistant (OTA), care and compassion are at the heart of your role. You will be part of an interdisciplinary team focused on providing compassionate quality care and achieving positive outcomes for your patient population. You will deliver home-based therapy to patients as directed by the attending physician and by following the order sets and goals created by the Occupational Therapist. This role involves interacting with patients' families, providing a rewarding privilege to be part of their recovery journey. Home care offers a real-world perspective on what truly helps patients restore their health. Post-acute care is crucial for improving health outcomes, as we care for patients in their homes, where they spend most of their time. This unique position allows us to observe factors invisible to primary care or hospital physicians, enabling tailored care. As healthcare shifts from hospital-centric models, our work in home care becomes increasingly vital. Our success stems from engaging with patients, coordinating their transition from facility bedside back to home, and ensuring their care plan is appropriate. We support complex, high-acuity, medically-at-risk aging patients, finding this challenging group the most rewarding to serve. We partner with health systems to create a seamless care environment, ensuring patients receive excellent care in the setting that best meets their needs.

Requirements

  • Occupational Therapist Assistant (OTA) with current license in the state of employment
  • Minimum one-year experience as a OTA in an acute care setting
  • Valid driver's license and auto insurance in your name as a driver
  • Capable of all physical demands

  • Home care experience preferred

Responsibilities

  • Provide comprehensive skilled services as ordered by the attending physician.
  • Educate patients and their family members and ensures the safety of the patient.
  • Accountable for completing accurate documentation and remaining compliant with all legal rules and regulations.
  • Provide skilled occupational therapy as ordered by the attending physician and observe and monitor patient conditions and notify the Occupational Therapist of any changes in the patient’s condition.
  • Assesses functional problems and provides intervention and, in conjunction with the occupational therapist works toward goals to improve patient functional status.
  • Counsel patient and family on the disease/injury and how to manage in all phases of occupational therapy.
  • Instruct patient and family members on proper use of equipment such as wheelchairs, braces, walkers, crutches, canes, and other prosthetic/orthotic devices.
  • Completes all clinical documentation following agency protocol and Medicare/Federal guidelines.
  • Understand and follow agency policies, procedures, rules, and regulations and communicate changes in schedule/availability to schedulers or supervisors.
  • Attend in-service training and mandatory agency meetings.
