Check out our website: Home - Fortis Job Summary: Fortis Health is seeking a skilled and compassionate Certified Occupational Therapy Assistant (COTA) to provide in-home therapy services under the supervision of a Registered Occupational Therapist (OT). This role delivers direct patient care based on the established plan of care, monitors patient progress, and supports patients and families in achieving safe independence at home. Positions Available: PRN Part-Time Full-Time

Requirements

  • Graduate of an accredited Occupational Therapy Assistant program (required).
  • Current Certified Occupational Therapy Assistant (COTA) license in the state of Indiana (required).
  • Minimum of 1 year of experience in a hospital, facility, or home health setting; home health experience (preferred).
  • Valid driver’s license, reliable auto insurance, and personal transportation (required).

Responsibilities

  • Provide occupational therapy interventions in accordance with the physician-approved plan of care developed and supervised by the Registered OT.
  • Deliver therapeutic activities and self-care training designed to restore function and improve independence.
  • Implement orthotic use and adaptive strategies as directed by the OT.
  • Observe, document, and report patient responses to treatment and any changes in condition to the OT and/or Director of Rehabilitation (DOR) or Director of Nursing (DON).
  • Educate patients, families, and caregivers on exercise programs, strengthening techniques, transfers, positioning, and activities of daily living.
  • Assist the OT with ordering, fabricating, or recommending adaptive and assistive equipment.
  • Participate in client care conferences and interdisciplinary team meetings to ensure coordinated, high-quality care.
  • Maintain accurate, timely, and compliant documentation in accordance with agency policies and regulatory requirements.
  • Participate in in-service training and continuing education to remain current with occupational therapy standards and best practices.
  • Support staff education initiatives as requested.
