About The Position

Pleasant Hill Post Acute is looking for a dedicated and energetic Certified Occupational Therapist Assistant (COTA) to join our premier in-house therapy team. Requirements

• A valid, current California Occupational Therapist Assistant (COTA) License.
• A compassionate, patient-centered approach to therapy.
• Strong communication skills and a desire to learn and grow within a skilled nursing setting.

Responsibilities

• Implement treatment plans as directed by the OT and physician to help patients master activities of daily living (ADLs).
• Maintain meticulous records, including daily treatment notes and weekly progress reports, in compliance with OT Board and payer requirements.
• Communicate effectively with supervisors and the broader health team regarding patient progress and discharge goals.
• Instruct nursing staff and families in maintenance programs and caregiver training to ensure a safe transition home; assist in training Restorative Aides.
• Assist with the cleaning and maintenance of the treatment area and report equipment issues to ensure a safe, high-functioning environment.
• Build positive, effective relationships with residents, families, and colleagues.
