About The Position

The patient care assistant assists in patient care delivery, clerical functions and provides unit support through a variety of functions. This role involves upholding Kaiser Permanente's Policies and Procedures, providing basic nursing care needs to assigned patients, and making visual observations to report abnormalities in accordance with established nursing procedures and policies. The assistant documents patient observation and direct care information in patients' medical records and works as a team member with assigned RNs and other healthcare workers to provide care. The position requires the ability to organize time and prioritize assignments effectively.

Requirements

  • Certified Nurse Assistant with 1000 hours recent acute clinical or skilled nursing facility experience within the last three years.
  • Basic Life Support certification.
  • Current Certified Nurse Assistant license in California.

Responsibilities

  • Uphold Kaiser Permanente's Policies and Procedures, Principles of Responsibilities and applicable state, federal and local laws.
  • Provide basic nursing care needs to assigned patients and make visual observations reporting abnormalities.
  • Document patient observation and direct care information in patients' medical records.
  • Take assignments accurately and work as a team member with assigned RN and other healthcare workers.
  • Visit each assigned patient at the beginning of the shift/assignment and report observations to R.N.
  • Make rounds on assigned patients at least every two hours, observing and reporting changes to the R.N.
  • Use the Multidisciplinary Plan of Care as a guide for individualized patient care and teaching as directed by the R.N.
  • Identify discharge barriers and communicate to R.N.
  • Assist with or provide activities of daily living (ADLs) including baths, linen change, dressing, feeding, grooming, and hygiene.
  • Collect simple specimens and record specimen collection.
  • Reinforce pulmonary hygiene including use of incentive spirometer and set up oxygen.
  • Administer minor treatments and procedures such as EKG test administration and non-sterile dressing changes.
  • Perform blood glucose monitoring and provide bed exercises.
  • Document care provided including oral intake, output, vital signs, weights, and measurements.
  • Transport patients for transfer, discharge or other needs as directed.
  • Provide post-mortem care and maintain cleanliness and organization of assigned work areas.
  • Monitor patient care environment for cleanliness and safety.
  • Remove faulty equipment from service and report to appropriate personnel.
  • Participate in emergency procedures and respond to codes as appropriate.
  • Establish and maintain courteous relations with the public, patients, and other personnel.
  • Assist in orienting new personnel and maintain patients' charts accurately.
  • Participate in staff meetings, nursing committees, and in-service programs.
