About The Position

The Home Health Certified Nursing Assistant provides direct and indirect care to patients in their residence as directed by the plan of care and under the supervision of a Registered Nurse in accordance with the philosophy, goals, and objectives of Cozad Community Health System. This description intends to describe the general nature and level of work performed by employees assigned to this job. It is not intended to include all duties, responsibilities and qualifications. Requirements

  • High school diploma or general education degree (GED); or one to three months related experience and/or training; or equivalent combination of education and experience.
  • Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals.
  • Ability to write routine reports and correspondence.
  • Ability to speak effectively to patients, visitors and employees.
  • Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals.
  • Ability to compute rate, ratio, and percent and to draw and interpret bar graphs.
  • Ability to apply common sense understanding to carry out detailed but uninvolved written or oral instructions.
  • Ability to deal with problems involving a few concrete variables in standardized situations.
  • Current Certified Nursing Assistant licensure for the State of Nebraska
  • Basic Life Support (CPR)
  • Valid Driver’s License

Nice To Haves

  • Electronic Health Records experience preferred.

Responsibilities

  • Participates in, monitors, and implements the patient’s plan of care.
  • Recognizes changes in the patient’s condition and notifies the primary RN in a timely manner.
  • Demonstrates expertise in provision of daily activities for patients and delivers such care in a professional manner.
  • Provides care that includes, vital signs, positioning, transferring, toileting, nutrition, ambulation, bathing, grooming, dressing/undressing, hygiene procedures, and linen changes.
  • Performs and maintains documentation.
  • Follows all HIPAA guidelines and promotes and ensures patient safety.
  • Special projects and other duties as assigned.
