About The Position
Sangre de Cristo Community Care is seeking a Per Request Needed Certified Nursing Assistant to join their dynamic inpatient hospice team in Pueblo, Colorado. The organization is a leader in Southern Colorado, dedicated to enhancing the quality of life and maintaining the integrity of individuals and their families in need of supportive or end-of-life services through symptom management, support, comfort, and compassion. The CNA is an integral member of the interdisciplinary team, providing patient care under the direction of an RN or Case Manager. This role actively contributes to the development and implementation of the patient’s Plan of Care, delivering a full range of personal care during regular visits to patients' homes and institutions, in adherence to hospice standards and regulatory scope of practice. The CNA assists patients with personal hygiene, ambulation, exercise, and nutritional support, while maintaining professional boundaries. This specific position is Part-Time or Per Request Needed for Weekend Coverage. Requirements
- Must have a current and clear CNA license in the state of Colorado
- At least 1 year experience as a CNA
- Must have a clear and valid Colorado Drivers License
- Ability to work a flexible schedule to include weekends and holidays
Nice To Haves
- Home Health or Hospice experience preferred
Responsibilities
- Provides patient care as directed by the RN or Case Manager
- Participates actively as a member of the IDT to assist in the development and implementation of the patient’s Plan of Care
- Provides a full range personal care through regular visits in patient's home and in institutions in accordance with hospice standards and scope of practice as defined with regulatory agencies
- Provides assistance with personal hygiene, ambulation and exercise and nutritional support for patient respecting personal and professional boundaries
