About The Position
THIS POSITION WILL BE RESPONSIBLE FOR PROVIDING CARE IN OUR FACILITIES SUMMARY: Works cooperatively with other team members to effectively implement the Hospice Plan of Care for patients.
Requirements
- Must be empathetic and responsible, able to use time effectively, be able to accept supervision, relate well to patients and understand and accept the hospice philosophy
- Current South Carolina Nurse Aide Certification
- Demonstrate competency in skills needed to perform job duties
- Ability to function as part of a team
- Ability to embrace the mission and culture of the organization
Responsibilities
- Assist patients with baths, care of mouth, skin and hair
- Assist patients with elimination functions
- Help patients in and out of bed; assisting with ambulation
- Perform such incidental household services as are essential to patient health care
- Report any changes or concerns regarding patients' conditions and/or family situations to the registered nurse
- Maintain effective working relations with patients, families and members of hospice interdisciplinary team
- Perform other related duties as assigned
- Document all aspects of care provided according to the policies established for the Hospice/HomeCare program and the individual patients care plan
- Participate in organization sponsored events
- Subject to a post employment background check and drug screening as a condition of employment.
