About The Position

For over 40 years, North Hill, a not-for-profit senior living community located in Needham, Massachusetts, has provided vibrant living opportunities to people 65+ by offering a host of hospitality, wellness and health care services. The Care Partner will contribute to a supportive and cooperative working environment, the well being and success of all customers by being positive, flexible and willing to help. The individual will partner with residents to provide resident centered care including ADL’s and the preparation and serving of meals. This is in Assisted Care/Enhanced Living. Services may include, however are not limited to, personal care, light housekeeping, meal preparation and delivery, companion care, and transport on the North Hill campus. Follows Medicare requirements and regulations when conducting certified home visits, and follows all agency policy and procedure for care and services provided to all patients/residents. Follows a written plan of care when providing direct patient care, and ensures that training has been conducted by the Wellness Nurse. Completes all required annual education on a timely basis Adheres to required policies and procedures related to confidentiality of personal health information of patients as required by HIPAA standards for medical information. Participates in interdisciplinary team meetings and provides input for care coordination and collaboration. Participates in department meetings, as required. May participate in the clinical orientation of Certified Nursing Assistants as requested by their manager. Complies with all HIPAA regulations, attends new team member orientation and completes all required annual education and training.

Requirements

  • High School diploma or equivalent.
  • Certified Nurse Assistant (CNA) course of study that is documented by an organization approved through the Commonwealth of Massachusetts.
  • Must be able to communicate effectively with Residents, Team Members and Visitors and must be able to communicate effectively in English.
  • Must exhibit tact and patience when dealing with challenging situations.
