This role involves providing continuous bedside presence during a patient’s active transitioning phase, ensuring they are not alone and are supported with dignity, peace, and comfort in their final hours. The Certified Nursing Assistant will perform personal care services including bathing, dressing, oral care, incontinence care, and gentle repositioning to maintain comfort and skin integrity. They will offer compassionate companionship, therapeutic presence, and emotional reassurance to patients throughout the dying process, and create and maintain a calm, quiet, and respectful environment. The position also includes providing respite and supportive presence for family members and caregivers, offering emotional support and guidance during the active dying phase, and observing and promptly reporting changes in the patient’s condition to the Registered Nurse Case Manager. Accurate documentation of care, safe use of durable medical equipment, and performing postmortem care are also key aspects of this role.
