About The Position

Perform personal care for the patient including bathing, dressing, toileting, and feeding according to the hospice aide plan of care. Document care provided according to the Care 360 Hospice policy. Report any deviations from the plan of care to the case manager. Use durable medical equipment safely Take vital signs when issued as part of the plan of care Foster cooperative effort among personnel by understanding the functions of other persons involved in patient care and by active participation in team and staff conferences. Assist with ambulation, transfers, and range of motion if documented on the hospice aide plan of care. Participate in QAPI activities Participate in on-site monthly educational meetings

Requirements

Licensed Nursing Assistant in the state of practice (CNA)
CPR/BLS Certified
Prior experience working in an AL/LTC/SNF facility or Home Health REQUIRED
Enjoy working with Senior Citizens, Positive Attitude, Professional, Responsible & Dependable
Must be willing to travel
Valid driver's license and Auto insurance Coverage with reliable transportation

Hospice experience preferred
