Requirements

  • Valid CNA certification
  • Experience with EMR and EHR systems
  • Knowledge of medical terminology and documentation
  • Familiarity with HIPAA regulations
  • Previous experience in patient care and creating care plans
  • You consistently follow universal precautions, including proper use of personal protective equipment (PPE) to minimize risk and maintain a safe environment.

Nice To Haves

  • CPR certification (preferred)

Responsibilities

  • Deliver Personalized, Dignified Care
  • Support elders with daily living needs like grooming, bathing, mobility, oral hygiene, toileting, feeding, and more – always with compassion, respect, and a personal touch.
  • Use each elder’s individualized care plan to guide your support and contribute valuable insights and observations to improve outcomes.
  • Help elders maintain their highest level of physical, mental, and emotional wellness through attentive, person-centered care.
  • Preserve privacy, dignity, and confidentiality in every interaction.
  • Foster a Safe, Comfortable, and Homelike Environment
  • Prepare and serve nutritious meals and snacks, taking into account dietary needs and preferences.
  • Perform light housekeeping, personal laundry, and keep the home clean and welcoming.
  • Assist with making beds, organizing spaces, and creating a warm, homelike atmosphere that encourages belonging.
  • Safeguard personal items like glasses, dentures, clothing, and keepsakes – because the little things matter.
  • Be a Key Part of a Self-Managed Team if You Work in One of Our 6 Green Houses
  • In our Green Houses, you will work within a collaborative, empowered team, rotating roles such as Care Coordinator, Housekeeping Coordinator, Food Coordinator, and more.
  • Participate in regular house meetings, share ideas, and help solve challenges together as a team.
  • Use your cooking talents to nourish, delight, and connect with our residents.
  • Take ownership of tasks and responsibilities while being supported by your team.
  • Stay Attuned to Elder Well-Being
  • Observe and report any changes in condition or behavior to the clinical team to ensure timely interventions.
  • Monitor skin integrity, turn and reposition immobile elders, and apply moisturizers or ointments as needed to prevent pressure injuries.
  • Respond to call bells and elder requests promptly and courteously – because every moment counts.
  • Support Clinical and Restorative Health
  • Take vital signs, assist with weight checks, and apply prescribed creams or treatments.
  • Assist with restorative care activities that promote strength, mobility, and independence.
  • Attend care team meetings and help shape personalized care strategies.
  • Use proper lifting techniques and safety protocols to support safe mobility and transfers.
  • Keep Learning, Growing, and Leading
  • Complete all required training, in-services, and competencies to stay current and confident in your role.
  • Help onboard new staff, sharing your knowledge and fostering a culture of support.
  • Participate in ongoing educational opportunities to grow professionally and personally.
  • Bring Joy and Engagement to Daily Life
  • Plan and lead fun and meaningful activities – from quiet moments to lively group events.
  • Coordinate special outings and connect elders with community groups, volunteers, and visiting programs.
  • Contribute to the monthly activity newsletter and ensure elders are informed and involved.
  • Be a Guardian of Rights, Safety, and Respect
  • Understand and uphold resident rights, including freedom from abuse, neglect, or restraint.
  • Report concerns immediately to leadership and follow established policies to ensure elder safety and protection.
  • Practice exceptional infection control – including hand hygiene, PPE use, and safe material disposal to maintain a healthy environment for all.
