About The Position

A Home Health Aide (HHA) provides direct patient care to patients under the direction of the RN and according to the Aide Plan of Care (POC). Responsibilities include assisting patients with self-administered medications, taking vital signs, maintaining personal hygiene, assisting with feeding, toileting, mobility, and documenting care provided. The HHA also practices acceptable infection control principles and assists with household duties as needed.

Requirements

• High school diploma or equivalent.
• Completion of Certified Nursing Assistant or Certified Home Health Aide Program within the last 24 months.
• Must meet applicable state certification requirements.
• A valid driver's license, auto insurance, and reliable transportation are required.
• Must be in good standing on the HHA Registry (if applicable).
• At least one year experience in the last 24 months as a Home Health Aide or Certified Nursing Assistant.

Responsibilities

• Provides direct patient care under the direction of the RN and according to the Aide Plan of Care (POC).
• Assists the patient with self-administered medications.
• Takes accurate temperature, pulse, and blood pressure measurements.
• Helps patients maintain good personal hygiene.
• Assists in feeding patients and communicates basic principles of nutrition.
• Assists with patient toileting and catheter bag changes.
• Provides skills to assist with patient mobility, exercises, and transfers.
• Documents interactions with patients and caregivers appropriately.
• Practices acceptable infection control principles.
• Assists with household duties including light laundry and meal preparation.
