About The Position

Under the supervision of an RN or LPN, the certified nursing assistant (C.N.A.) performs resident-centered, evidence-based nursing care activities to meet the physical and psychological needs of the resident. Through collaborative practice with all members of the care team, is responsible for resident outcomes that meet the high quality of care provided by ThedaCare. Contributes to the goals of the department by being accountable for the delivery of compassionate, safe care and demonstrated commitment to continuous growth and professional development.

Requirements

  • Valid State of Wisconsin C.N.A. certification required
  • American Heart Association Healthcare Provider Basic Life Support (BLS) certification required.
  • Position requires float between Juliette Manor, Peabody Manor and Cherry Meadows
  • Ability to move freely (standing, stooping, walking, bending, pushing and pulling) and lift up to a maximum of fifty (50) pounds without assistance
  • Category 1: Job classification is exposed to blood borne pathogens (blood or bodily fluids) while performing job duties

  • 2 years of healthcare experience preferred.

Responsibilities

  • Performs delegated patient care activities using evidence-based practice and specific competencies for the resident population served.
  • Assists in meeting the care needs of the resident in collaboration with the care team to ensure quality, cost-effective, safe resident care.
  • Demonstrates clinical expertise in the provision of care in the long term and rehabilitative care setting of the skilled nursing facility.
  • Performs all functions of the certified nursing assistant which are age appropriate, developmentally sensitive, and culturally specific.
  • Respectfully addresses situations or decisions to ensure resident concerns and needs are met.
  • Identifies ways to improve the residents experience of care, streamline care processes, and lower costs to improve resident, family, and team member satisfaction.
  • Identifies and escalates resident risk to prevent crises.
  • Demonstrates professional and therapeutic relationships with residents, families, team members, and providers.
  • Reinforces resident education to improve care outcomes and safe resident practices.
  • Performs and documents resident cares including personal hygiene, mobility, activities of daily living, and ensures the care environment promotes resident safety.
  • Checks resident vitals, height, and weight as needed based on care plan.
  • Obtains and reports patient intake and output as directed.
  • Collects body fluids for testing and culture as delegated by RN or LPN (examples: urine or stool samples).
  • Provides resident care as defined within the nursing care plan, provider orders, and within the scope of the C.N.A. role.
  • Obtains point of care blood sugars as ordered. Apply specific creams and lotions as delegated by RN or LPN.
  • Supports meeting resident dietary needs through gathering resident meal orders, water pass, snack pass, meal tray pass and set up, and assist with feeding as indicated in care plan.
  • Ambulates residents in accordance with care plan.
  • Proficient in use of mechanical transfer devices such as slide boards, hoyer lifts, sit to stand devices, etc.
  • Proficient in use and care of oxygen, including filling containers and appropriate application on resident.
  • Supports healthy urinary and bowel function by understanding and following toileting programs, providing routine indwelling urinary catheter care, and ostomy care as needed.
  • Supports skin integrity by proper positioning and repositioning of all residents, assures specialty products are in place as needed (specialty mattresses, wheelchair cushions, etc.).
  • Performs restorative therapy for long term care residents as directed by therapy restorative care plan.
  • Assures home like environment within resident rooms by making beds, emptying trash as needed, and respecting resident personal belongings.
  • Maintains C.N.A. certification as required by regulatory agencies.
  • Collects clinical data and information on residents and reports to care team.
  • Must achieve 12 hours of in-service education annually as required by federal regulations (SNF)
