About The Position

Requirements

• Recent experience in acute care and/or long-term care facility is preferred.
• Ability to clearly communicate with other members of the healthcare team.
• Ability to understand and follow directions from the healthcare team.
• Knowledge of patient and healthcare safety standards and regulations (e.g., falls, skin breakdown, suicide prevention).
• Infection prevention and transmission-based precautions knowledge.
• Understanding of clerical duties and office-based technology.
• Knowledge of team dynamics.
• Ability to collect and document patient data (e.g., vital signs, intake/output, changes in patient condition).
• Skills in basic hygiene and activities of daily living care.
• Knowledge of feeding, hydration, and nutrition.
• Understanding of bowel and bladder elimination.
• Ability to assist with ambulation and patient mobility.
• Familiarity with clinical technology.
• Compassionate, relationship-based approach in care activities.
• Accountability for completion of assigned tasks.
• Ability to escalate concerns via the chain of command.

Nice To Haves

• BLS Basic Life Support certification from the American Heart Association (preferred).
• State Certified or eligible to take the state test within 120 days of employment.

Responsibilities

• Provides basic patient care including care and comfort, recording vital signs, personal care and hygiene, and mobility.
• Acts as a liaison between the patient and nurse to report changes or concerns.
• Provides high-level customer service to patients, their families, visitors, and employees.
• Ensures accurate, precise, and timely documentation for patient care when applicable.
• Utilizes professional verbal and written communication skills at all times.
