About The Position

Community Memorial Health System was established in 2005 when Community Memorial Hospital in Ventura merged with Ojai Valley Community Hospital. This position performs resident/patient care activities and related services necessary in caring for the personal needs, safety, and comfort of residents/patients as assigned. Assists in providing a positive physical, social, and psychological environment. The CNA II relates positively, effectively, and appropriately with residents/patients, families, community members, volunteers and other facility staff. Successful incumbents have fast and adaptive problem-solving skills who value collaboration and teamwork. This position has an in-depth understanding of patient care policies and regulations and is comfortable working with specific populations of patients, including elderly patients and those with special needs.

Requirements

  • Current Certified Nursing Assistant (CNA) certification.
  • Current Basic Life Support (BLS) certification.

Nice To Haves

  • High school Diploma or equivalent (GED).
  • Six (6) months of CNA experience performing Nursing/Patient Care duties in an acute care setting or skilled nursing facility.

Responsibilities

  • Assists with resident/patient care needs and comforts including but not limited to: Assists or gives oral hygiene and mouth care.
  • Assists with resident/patient care needs and comforts including but not limited to: Assists with bathing functions and dressing/undressing as necessary.
  • Assists with resident/patient care needs and comforts including but not limited to: Assists with nail care, shaving and hair care.
  • Assists with resident/patient care needs and comforts including but not limited to: Assists with lifting, turning, positioning and transporting residents.
  • Assists with resident/patient care needs and comforts including but not limited to: Makes both occupied and unoccupied bed (as assigned).
  • Assists with resident/patient care needs and comforts including but not limited to: Keeps residents/patients dry (i.e. change gown, clothing, linen, etc.) when they become soiled or wet.
  • Assists with resident/patient care needs and comforts including but not limited to: Assists resident/patient with bowel or bladder functions.
  • Assists with resident/patient care needs and comforts including but not limited to: Checks resident/patients frequently to assure that their personal care needs are being met.
  • Assists with nursing functions as follows: Measures and records vital signs.
  • Assists with nursing functions as follows: Weighs and measures residents/patients.
  • Assists with nursing functions as follows: Collects Specimens (i.e. urine, stool, sputum).
  • Assists with nursing functions as follows: Prepares and gives enemas.
  • Assists with nursing functions as follows: Provides daily indwelling catheter care.
  • Assists with nursing functions as follows: Performs diabetic urine testing.
  • Assists with nursing functions as follows: Turns bedfast residents/patients at least every two hours.
  • Assists with nursing functions as follows: Provides daily range of motion exercises.
  • Assists with nursing functions as follows: Assists with care of dying residents.
  • Assists with nursing functions as follows: Maintains accurate intake and output records.
  • Assists with nursing functions as follows: Noting observations of patient behavior, including complaints, or physical symptoms to nurses.
  • Performs food service functions including: Prepares resident/patients for meals (i.e. take to bathroom, wash hands, oral care, take to and from dining room, etc.).
  • Performs food service functions including: Serves food trays. Assists with feeding as indicated.
  • Performs food service functions including: Records the residents/patients food fluid intake.
  • Performs food service functions including: Serves between meal and bedtime snacks.
  • Performs food service functions including: Keeps resident’s/patient’s water pitchers clean and filled with fresh water on each shift.
  • Performs food service functions including: Performs after meal care (i.e. clean residents/patients hands, face, clothing, brush teeth, take to bathroom, etc.).
  • Performs food service functions including: Notes and reports changes in resident/patient eating habits, especially those on diabetic diet.
  • Assists with keeping nursing and resident/patient care area neat, sanitary and orderly.
  • Floats to the OVCH Acute Care Facility four (4) times each year.
  • Appropriately records and charges for supplies.
  • Performs other duties as assigned.
