As a CNA, you’ll provide hands-on care to residents in our skilled nursing facility, including those in our long-term memory care and short-term rehab units. The Terrace is a 115-bed skilled nursing facility in Waukegan, dedicated to providing high-quality care to older adults through short-term rehabilitation and long-term memory care. Our residents come to us following hospitalization, surgery, or illness, or because they need ongoing support related to dementia or chronic conditions. Clinical care at The Terrace is resident-centered and team-driven. Nurses, therapists, social services, and support staff work closely together to promote recovery, maintain function, and preserve dignity. We focus on safe transitions of care, individualized treatment plans, and meaningful outcomes for each resident. At The Terrace, clinicians are respected as professionals. We currently provide comprehensive management services to several inpatient healthcare centers throughout Northern IL & the Chicagoland area. These settings include Long-term Care Skilled Nursing (SNFs), Short-term Care Rehabilitation, Intermediate Care (ICFs), and Behavioral Health Service (SMHRFs) centers. Requirements
- Certified Nursing Assistant (CNA) certification in IL
- Prior experience in a hospital or a skilled nursing facility is ideal
- Electronic medical records experience (Point Click Care is helpful)
Responsibilities
- Assist residents with daily living activities such as bathing, dressing, and mobility
- Provide personal care with dignity, respect, and empathy
- Support residents in memory care and rehab units with specialized needs
- Monitor and report changes in residents’ condition to nursing staff
- Build trusting relationships with residents and their families
