Community Care is hiring Full-Time Certified Nursing Assistants/CNAs for the Day Shift at our Sacred Heart site in Milwaukee, WI. Requirements

Education: High School diploma or GED preferred
Experience: Six months of experience providing personal care services for elderly or disabled adults in a group home, assisted living or long-term care environment and experience working with individuals with dementia.

Responsibilities

Provide various care services necessary for attending to the personal needs
Support the health of Community Care program members
Serve as coordinator of care for scheduled weekend and holiday shifts
Ensure care and service delivery are in conformance with regulations, policies, procedures, and standards, established by Community Care
