About The Position

Pleasant Hill Post Acute is seeking compassionate and dependable Certified Nursing Assistants (CNAs) to join our night-shift team. As a NOC shift CNA, you are the watchful guardian of our residents' comfort and safety during the overnight hours. Requirements

• A current and valid California Certified Nursing Assistant (CNA) Certification.
• A passion for geriatric care and a commitment to night-shift excellence.
• Strong communication skills and a reliable, "team-first" work ethic.

Responsibilities

• Provide routine daily nursing care in accordance with individualized care plans and supervisory direction.
• Act as the "eyes and ears" of the clinical team, promptly reporting any changes in resident condition to the Nurse Supervisor or Charge Nurse.
• Maintain descriptive and accurate records on flow sheets, charts, and notes to ensure continuity of care.
• Safely identify residents before treatments or meals and immediately report any accidents or incidents.
• Prepare rooms for new admissions, greet and escort residents, and assist with personal inventory during transfers or discharges to ensure a seamless experience.
• Safeguard confidential health information (PHI) and perform nursing procedures only within your scope of training and authorization.
