About The Position

We are looking for a compassionate, certified Home Health Aide for the East Valley to deliver personal care and support to patients in their homes, following the direction and supervision of a registered nurse (RN). This position is perfect for someone who takes pride in working closely with seniors, pays careful attention to detail, and is dedicated to helping individuals live safely and independently. This is a PRN position and serves patients in the Phoenix Central and East Valley, offering a visit rate plus mileage reimbursement.

Requirements

• Current Certified Nursing Assistant (CNA) / Home Health Aide (HHA) certificate in the State of Arizona.
• Must meet Medicare Conditions of Participation for Home Health Aide training and competency.
• At least 18 years of age.
• Ability to read, write, and communicate effectively in English.
• Compassionate, dependable, and able to maintain a warm, caring manner.
• Physical ability to perform lifting, bending, standing, and other essential job functions.

Responsibilities

• Provide personal care, bathing, and hands-on assistance to patients as assigned.
• Assist with ambulation, exercises, and simple procedures as directed (e.g., range of motion exercises).
• Support patients with self-administered medications as assigned.
• Perform light household tasks essential to the patient’s care plan.
• Complete visit documentation accurately and on time.
• Monitor and report changes in the patient’s condition to the RN.
• Participate in in-service training, continuing education, and patient care conferences.
