About The Position

Under the direct supervision of a licensed nurse or other qualified healthcare professional, the Certified Nursing Assistant (CNA) provides high-quality direct patient care in a Skilled Nursing Facility (SNF). This role supports patient comfort, safety, and overall well-being while contributing to a collaborative, patient-centered care environment.

Requirements

• Graduate of an approved Certified Nursing Assistant (CNA) training program
• Certified Nurse Aide listed on the Nurse Aide Registry (Prometric)
• Active New York State CNA Certification (Primary Source Verification required)
• Current AHA Basic Life Support (BLS) certification
• Minimum of one (1) year of experience in a nursing home or skilled nursing facility
• Strong teamwork, communication, and patient care skills

Responsibilities

• Assist patients with activities of daily living (ADLs), including bathing, dressing, grooming, and mobility
• Perform assigned clinical procedures such as taking vital signs, changing simple dressings, and administering enemas as directed
• Accurately record and document personal care provided and clinical observations
• Prepare patients for examinations by measuring temperature, pulse, respiration, height, weight, and other required metrics
• Assist during examinations and accurately record findings in patient charts
• Observe, report, and document changes in patients’ physical condition, mental status, and behavior
• Participate in nursing and interdisciplinary team conferences to support care planning
• Assist with recreational, remotivational, and resocialization activities as appropriate
• Set up, maintain, and clean examination and treatment areas
• Support patient admissions and assist with visitor coordination
• Safeguard patient clothing and personal belongings
• Assist patients during meals and provide feeding assistance as needed
• Change linens, make beds, and maintain cleanliness of patient rooms and common areas
• Provide postmortem care respectfully and in accordance with facility policies
