About The Position

Responsibilities: Under the direction of a Registered Nurse or Case Manager, will be required to: Recording pertinent information pertaining to the patient’s care. Reminding patients to take medication at the appropriate time. Planning and preparing meals according to specific dietary requirements of the patient, and if necessary, feeding the patient. Assisting the patient with therapy exercises under the direction of a Registered Therapist. Assisting or performing personal care including hair care, oral hygiene, bathing, grooming, and dressing. Assisting with ambulation with or without mechanical aids. Assisting with routine bodily functions: Example, toileting. Maintaining a safe, clean and healthy environment through light housekeeping includingchanging bed linens, dusting and vacuuming, cleaning kitchen and bathroom, and laundry. Providing companionship and stimulation for the patient including reading, walks, etc.; and accompanying the patient to doctors or other appointments. Grocery shopping or other errands when needed. Performing other housekeeping tasks as indicated in the care plan. Taking patient’s vital signs and recording input and output. Observing and reporting changes in patient’s condition to the registered nurse. Accurately preparing daily records and submitting them to the office by date due. Participating in in -service education programs. Do not accept any form of financial support from a patient or family members.

Requirements

  • Must have successfully completed a CNA training course; or have successfully passed anequivalency exam and posses written evidence of such completion.
  • Copy of CNA certification (at hire)
  • Copy of current Tb/PPD test results (at hire and every 2 years)
  • Copy of Hep -B Vaccination dates or declination statement (at hire)
  • Copy of current BLS / ACLS certification(s) or other specialty certifications asnecessary (at hire and current)
  • Completed Skills Competency Assessment (at hire)
  • Copy of Annual Medical Examination (at hire and current)
