About The Position

Requirements

• Active CNA certification in the state of Wisconsin
• A strong passion for providing high-quality patient care
• Reliability and punctuality - attendance is key
• Ability to work well in a team-oriented environment

Responsibilities

• Provide quality care in a supportive environment
• Offer individualized attention to help residents maintain the highest practical physical, mental, and psychosocial well-being
• Assist residents with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and feeding
• Support residents' needs related to incontinence care, mobility, range of motion, and communication
• Maintain skin integrity by preventing pressure ulcers, skin tears, and other injuries through proper care and repositioning
• Perform assigned tasks from the floor nurse, including vital signs, weighing residents, applying creams/ointments, and collecting specimens
• Follow individual care plans and contribute to the care planning process by reporting resident needs, preferences, and behavioral changes
• Document medical records accurately while maintaining strict confidentiality
• Ensure infection control and safety protocols are followed at all times
• Promptly and courteously answer and respond to call lights
• Assist with new admissions, re-admissions, and resident transfers within the facility
• Communicate effectively and interact tactfully with residents, families, visitors, and team members
• Attend and participate in departmental meetings and training sessions
