About The Position

Join our 16 bed Transitional Care Unit, providing short term skilled nursing and strengthening care to help patients safely transition back to the community following acute illness. Responsibilities • Provide direct resident care under the direction of the RN and LPN • Assist with activities of daily living including bathing, dressing, feeding, toileting, ambulation, and transport • Respond promptly to resident needs and call lights • Perform routine tasks such as vital signs, weights, and intake and output • Assist with feeding, deliver nourishments, and fill water pitchers • Observe and report changes in resident condition to licensed nursing staff Safety and Clinical Support • Apply, monitor, and remove restraints per physician and facility protocols • Use diversional activities to reduce or prevent restraint use • Document care accurately in the medical record • Follow all safety, infection control, and confidentiality policies • Respond to emergent situations within the scope of the nursing assistant role • Maintain current CPR certification Unit and Team Support • Maintain a clean, organized, and safe environment • Assist with linen changes, equipment cleaning, and room organization • Stock supplies and report equipment malfunctions • Support unit operations including call lights, phones, meal trays, and transport • Communicate effectively and maintain professional relationships • Accept feedback and actively work to improve skills and performance

Requirements

  • High school diploma or equivalent preferred.
  • Knowledge of body mechanics and transfer techniques.
  • Ability to effectively communicate both orally and in writing.
  • Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR) within 30 days of hire
  • Certified Nursing Assistant (CNA)
  • Act 34
  • OAPSA
