Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. Assists the client with or provides personal care per the assigned plan of care. Personal care duties include: bathing (tub, shower, or bed), care of hair, skin, feet and mouth, toileting (bed pan, commode or toilet), dressing, transfer, ambulation, simple range of motion, assist with self-administration of medications, stand by assistance. Takes and records client temperature, blood pressure, pulse and respiration. Prepares and serves client meals. Documents activities performed in the client’s home at the time of the visit. Documents client concerns, affect and/or difficulties and shares findings with the Clinical Coordinator or Administrator. Organizes and implements his/her schedule to meet productivity expectations. Promptly communicates schedule changes to Agency and client. Completes and submits required paperwork accurately and on time. Maintains the set productivity standard of an average of four clients per day. Completes job responsibilities in a timely, safe and professional manner. Participates in the development of clients’ care plan. Participates in client case conferences according to home health agency and procedures. Represents the Agency in a professional manner. Performs other related duties as assigned.

Requirements

  • Certified Nursing Aide license in Colorado (If not certified, must have the ability to obtain the license.)
  • Current CPR certification
  • Current First Aid certification
  • Possession of a valid state driver's license with reliable transportation and applicable car insurance, if transporting patients
  • Proof of COVID-19 Vaccination or approved medical or religious exemption

  • Preferably experience in home health work.
