As a member of the Health Support Center (HSC) team, you'll support those that are in our facilities who are interfacing and providing care to our patients and community members to positively impact our mission of making communities healthier. This role offers an opportunity to work collaboratively in a supportive environment focused on providing quality care and support to individuals requiring temporary rehab assistance and LTC needs.

Requirements

• Current state NA I license
• Completed NA course from an accredited school
• Basic Life Support certification is required
• One year of experience preferred

Responsibilities

• Accurately identifies patient needs
• Provides daily care and support long-term care residents
• Promote comfort, dignity, and safety for all patients
