Position Overview: The CNA is responsible for providing residents with a high level of caregiving efforts and supporting medical staff in administering care to residents with various needs. CNA responsibilities include, helping residents bathe, maintain proper hygiene, feeding residents, ensuring residents take their medication and assessing vital signs. The caregiver consistently works with all neighborhood/house team members, led by resident and family preference to meet the needs and desires of all residents.

Requirements

  • High school diploma or equivalent required.
  • Graduate of a state approved program for nursing assistants; valid CNA license required.
  • Demonstrates proficiency at reading and understanding the Resident Plan of care or assignment
  • Understands quality of care objectives.
  • Understands quality of life objectives.
  • Competency evaluated, tested, and registered with the state of Kansas.
  • Must be a minimum of 16 years of age.
  • Must be able to hear and understand resident requests.
  • Must be able to read accurately and retain information that was read and apply it.
  • Must be able to lift and carry up to 50 pounds.
  • Able to withstand extreme temperatures, hot and cold.
  • Able to work long hours, including some evenings, weekends, holidays, and as needed.
  • Must be able to stand or sit for long periods (4 hours or more).
  • While performing the duties of this job, the employee is regularly required to talk and listen.
  • The employee is frequently required to sit; stand; walk; use hands to touch, handle or feel; and reach with hands and arms; lift and pull.

  • Spanish bi-lingual capabilities are a plus.

Responsibilities

  • Provides physical care to the residents of the neighborhood/house to meet their daily needs and enable them to function at the highest possible level.
  • Be the residents’ closest advocate, under the direction of a professional nurse.
  • Promote the residents’ psychosocial well-being through meaningful relationship building.
  • Works with the neighborhood/house team to meet other social needs of the resident and of the neighborhood/house.
  • Provide support, friendship, compassion, and kindness for each of the members.
  • Plan and facilitate social activities for residents of the neighborhood /house which provide meaningful ways to spend time as recreation, exercise, relationship building and fun.
  • Attend to the dietary and nutritional needs of the resident by establishing a pleasant and enjoyable dining experience.
  • Participates and leads as needed in the dining process, for example, help other team members serve residents breakfast, lunch and/or dinner and/or clean-up afterwards.
  • Builds home by respecting the resident’s room as their home and assists the resident in feeling in charge of everything that happens within.
  • Contributes to making the neighborhood/house a healthy and happy place, works together to get things done, facilitates the creation of homey spaces, sees the possibilities an creating a daily life that naturally facilitates pleasurable moments, laughter, and fun with lots of opportunity for a wide variety of meaningful relationships
  • Help maintain the neighborhood/house by maintaining a clean, safe environment as if this was your own home.
  • Care for the residents’ personal laundry.
  • Preparing each resident's room with necessary items: blankets, pillows, medical equipment, and tioletries.
  • Assist housekeeping by cleaning rooms, when needed (dusting, taking out trash, vacuuming).
  • Attends to all resident personal hygiene and personal care needs, including but not limited to grooming, hygiene, dining and nutritional, mobility, elimination, and safety needs.
  • Organizes all supplies necessary to complete the tasks.
  • Monitoring vitals and behavior and reporting them to the nursing and/or medical leaders.
  • Transferring residents with low mobility from wheelchair to bed and vice versa.
  • Turning or adjusting residents in bed to prevent bedsores.
  • Supports residents in self-care and provides care as necessary and as designated on the Plan of Care for those unable to care for their own needs.
  • Support the psychosocial needs of individuals and all members of the neighborhood/house through assistance and encouragement to engage in activities and social contacts through support, friendliness, and kindness.
  • Follows standard precautions, proper infection control, sanitation, and safety standards of practice in all work and activities.
  • Provides restorative therapy under the leadership of therapy professionals and in accordance with the resident’s functional maintenance plan.
  • Observes residents closely, identifying changing needs and conditions and reports immediately to nursing or leadership as appropriate.
  • Participates actively in the resident’s care planning and care conference, serving as the residents’ closest advocate in directing their care and honoring their preferences.
  • Consistently works well with all household team members, led by resident and family preference to meet the needs and desires of all household residents.
  • Maintains documentation.
