About The Position

Are you a compassionate caregiver with a heart for helping others? As a Certified Nurse Aide (CNA) in our day center, you'll play a vital role in ensuring our participants receive the highest quality care. From assisting with daily activities to providing comfort and companionship, your work will make a meaningful impact in the lives of those we serve. Primary Responsibilities: Communicates effectively with participants, families, staff and the staff of other agencies Participates in Interdisciplinary Team (IDT) meetings as directed Provides basic ADL nursing care to participants Documents observations, nursing care and participant activities on appropriate forms and in the medical record as appropriate Reports changes in the participant’s condition to the pertinent IDT member Active participation in the Quality Improvement Process (QIP) Clean facility equipment and maintain clean facility condition Reports on participant status or family concerns to the DCS Completes home visits when directed Abides by all established PACE of Southwest Michigan policies, rules and regulations The above is a summary of the position, it in no way states or implies that these are the only duties this position will be required to perform. Requirements

  • A current and valid Michigan CNA Certification (a must!)
  • The ability to read, write, understand, and follow directions like a pro.
  • A heart for helping others and the physical ability to lift, stand, bend, transfer, stretch, walk, push, and pull as needed.

Responsibilities

  • Communicates effectively with participants, families, staff and the staff of other agencies
