About The Position

Bluestem PACE is looking for a full-time Homecare CNA to join our team! The hours for this position are 8:00am - 4:30pm, though they may vary some days depending on the needs for scheduling. The primary responsibility of the Home Care CNA is to perform ADL's and activities within the scope of practice of a Certified Nursing Assistant such as clinical observation, clinical reporting requirements, participant laundry, participant care training and education. Assist in monitoring and reporting the health status and provide a safe environment for the participants. The Home Care CNA will be responsible to provide and assist participants with activities of daily living, restorative and supportive care. Monitor health status and provide a safe environment for participants. Function as a member of the interdisciplinary team on an as needed basis.

Requirements

• Must be at least 18-years of age.
• Must have current Kansas CNA Certification.
• Must have current driver’s license with proof of auto insurance and safe driving record.
• Must be knowledgeable of principles, practices, standards and techniques of a CNA.
• Must have basic knowledge in working with frail and elderly to support Activities of Daily Living and Incidental Activities of Daily Living.
• Must be able to establish and maintain effective working relationships with patients, co-workers, medical staff and the public.
• Must have the ability to react calmly and effectively in emergency situations.
• Must be able to work effectively with culturally, economically and educationally diverse populations.

Responsibilities

• Consult each day with the Center Director or RN to be updated on participant’s status, changes in care plan and to confirm scheduled visits.
• Assist participants in the home with personal hygiene such as skin, mouth, hair care and bathing.
• Assist with transfers in and out of bed and ambulation.
• Assist with prescribed exercise as instructed by the rehab staff or nursing supervisor.
• Prepare meals, including special diets as directed by the nursing supervisor and assist participants with meals and/or feeding when needed.
• Assist participants to the bathroom or in using commodes, bedpans or urinals.
• Assist participants with medications by cueing.
• Perform directed household services.
• Report changes in participant’s condition and needs to the Center Director or RN.
• Maintain the confidentiality of all company procedures and information about participants, clients or families.
• Handle potentially infectious specimens with appropriate biohazard precautions.
• Participate as a member of the interdisciplinary team on an as needed basis.
• Assist and/or supervise participants with activities of daily living as assigned or needed under supervision.
• Assist transportation staff with the loading and unloading of participants from vehicles.
• Provide Escort assistance on transportation vehicles when requested.
• Note changes in participant’s physical or emotional health and communicate this information to the Center Director or designee.
• Maintain participant’s medical record and fulfill charting and reporting requirements.
• Assist the transportation department with participants’ takes home supplies.
• Participate in and support Quality Improvement initiatives.
