About The Position

JOB SUMMARY Provides all necessary services needed to assist hospice patients with their personal care and activities of daily living. JOB DUTIES Provides direct care to hospice patients in the home. (Visit setting may be patient's physical home or SNF/ALF) Greets patients and their families courteously. Prioritizes patient care. Provides all identified needs of patients under the direct supervision of the Hospice RN, who operates under physician orders. Provides assistance with all ADLs. Supports the hospice philosophy. Provides family education while working with the interdisciplinary team. Provides emotional support for patients and their families. Provides bereavement contact to families after patient's death, with interdisciplinary team. Maintains a safe environment for patients and families. Demonstrates knowledge of emergency procedures. Prepares meals, snacks, and supplements as needed if requested by patients or families. Maintains close communication with RN supervisor regarding patient status. Notifies RN of any changes in patients' conditions in a timely manner. Performs other duties as assigned.
