About The Position

Responsible for providing Care Management (CM) services for patients following established standards and practices. CM is a comprehensive, patient-centered, and team-based approach to managing chronic conditions and improving health outcomes. The Certified Medical Assistant may assist with other Medical Assistant duties as assigned, displays responsible behaviors, communicates effectively to others, and functions as a member of the healthcare team under the direct supervision of the physician.

Requirements

Nationally recognized certification as Medical Assistant (Two years of medical assistant experience may substitute for certification).
Ability to communicate effectively with others in the healthcare team.
Understanding of medical terminology, anatomy and physiology, diagnostic procedures, pharmacology, and treatment assessments to the extent required to understand and accurately transcribe dictated reports.
Ability to translate medical abbreviations into their expanded forms.
Ability to learn and use all functions of electronic medical record software.
Ability to accurately enter data into a database, search for information, send and receive email and attachments.
Skills in applying and modifying the principles, methods, and techniques of professional nursing to provide ongoing patient care.
Ability to communicate effectively in written and verbal form, answer frequent telephone calls.
Ability to work as a team player.
Ability to work flexible hours.
Ability to multi-task.
Ability to handle a fast-paced work environment during entire time at work due to frequent interruptions.
Ability to prioritize multiple projects.
Ability to maintain strict confidentiality and exercise professionalism when interacting with staff, managers, supervisors, physicians, external customers, etc.
Ability to comply with HIPAA confidentiality standards when accessing or communicating patient information.
Ability to relate to people and different situations with good judgment and integrity under stressful situations.

Nice To Haves

Previous medical office experience.
EMR experience.
Personal experience in care management, preferably in chronic disease management or have been exposed to this type of care management during your training.

Responsibilities

Participates in the care of the patient, under the direct supervision of the physician and in collaboration with the healthcare team.
Greets and escorts patient to exam rooms.
Collects patient information and assessment data.
Obtains and records patient's vital signs: temperature, pulse, respirations, blood pressure, weight and height.
Performs chart review to obtain historical data.
Perform Wellness Visits.
Assists physician with procedures/treatments/examinations (preparation, positioning, supplies, equipment).
Maintains and uses principles of aseptic techniques and infection control when performing clinical duties and tasks.
Ensures that patient chart entries are made accurately and in a timely manner; forwards charts as appropriate.
Providing ongoing monitoring and follow-up of patients' progress and adherence to care plans.
Educating and empowering patients and caregivers to self-manage their conditions and access available resources.
Confirms and clarifies written orders prior to implementation of delegated tasks.
Performs delegated procedures, ie. injections, nebulizer treatment, etc.
Observes, records and reports patient's condition and reaction to drugs and treatments to physician.
Follows policies for infection control in handling and disposing of infectious/hazardous waste materials.
Appropriately obtains and manages specimens, including: labeling, form completion and storage awaiting transfer to laboratory.
Recognizes emergency situations and implements emergency procedures according to clinic guidelines.
Administers first aid, CPR, and maintains emergency equipment and supplies.
Provides appropriate patient specific education, and health information materials.
In close partnership with healthcare team, assures smooth and efficient patient flow and clinical operation.
Makes, receives and handles medically related phone calls as directed by physicians.
Orders, stocks, and maintains exam room supplies.
Assists receptionist in scheduling appointments as needed.
Ensures appointment preferences are given to patients in emergency situations.
Completes all logs as required by OSHA Regulations. (i.e. Refrigerator log recorded twice daily, Eye Wash Station log recorded daily, Crash Cart log recorded monthly, etc).
Count and record all injectable medications kept in locked cabinet and refrigerator daily.
Inspect all sample and injectable medications weekly and dispose of any expired drugs according to Imperial Health guidelines.
Notifies patients of test results; schedules follow-up appointments as needed.
Schedules CT scans, MRIs, EEGs, etc. according to physician's orders.
Supply patient with written instructions for preparation needed for any and all special procedures or surgeries.
Conducts Remote Patient Monitoring (RPM)'s for physicians.
Perform other tasks as assigned.
