About The Position

The PACE LIFE Program Home Health Aide provides and assists participants with activities of daily living, restorative and supportive care necessary to the preservation of the home environment. This position also reports the health status and provides a safe environment for the participants. The Home Health Aide ensures that patient care is in accordance with the plan of treatment assigned by the primary nurse, and complies with other established standards. This position supports organizational goals by providing quality customer service, participating in performance improvement efforts and demonstrating a commitment to team work and cooperation.

Requirements

  • High School diploma or equivalent
  • Successful completion of a home health aide training program meeting national standards
  • Home Health Aide certification in the State of New Jersey
  • BLS required within 30 days from date of hire or transfer
  • Valid NJ driver's license, valid registration and insurance
  • Safe driving record
  • Minimum 1 year working with the frail or elderly
  • Proficiency in Clinical Applications required for this position within 30-60 days from date of hire (after appropriate training)
