As a Certified Home Health Aide (HHA), you will provide care to clients in their home, following their plan of care. Job Description JOB TITLE: Certified Home Health Aide (HHA), Home Care (Per Diem) DEPARTMENT: Home Care LOCATION: Rochester Regional Health Home Care - 330 Monroe Avenue Rochester, NY 14607 SCHEDULE: Per Diem ATTRIBUTES Compassionate, patient focused and dependable professional Critical thinking skills, decisive judgment and the ability to work with minimal supervision Exceptional documentation and communication skills Ability to read, write and speak English, ability to follow verbal and written instructions RESPONSIBILITIES Patient Care & Service. Promote and restore patients' health by completing all activities listed in the care plan; collaborate with Community Health Nurse/Therapist; perform various activities such as personal care, light housekeeping, meal preparation and errands Communication. Maintain effective communication to convey patient health status, treatment plans and progress; communicate with office and supervisory staff regularly Documentation. Accurately and completely document client care on activity sheet Compliance. Adhere to required department and system protocols, HIPPA regulations, local, state, federal requirements EDUCATION: LICENSES / CERTIFICATIONS: HHA - Home Health Aide certification - New York State Education Department (NYSED)New York State Education Department (NYSED)New York State Education Department (NYSED), NYS DL - valid New York State Driver's License - New York State Department of Motor Vehicles (NYSDMV)New York State Department of Motor Vehicles (NYSDMV)New York State Department of Motor Vehicles (NYSDMV) PHYSICAL REQUIREMENTS: M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting. For disease specific care programs refer to the program specific requirements of the department for further specifications on experience and educational expectations, including continuing education requirements. PAY RANGE: $18.65 - $19.75 CITY: Rochester POSTAL CODE: 14607 The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components.
