We are seeking a Certified Home Health Aide (CHHA) or Certified Nursing Assistant (CNA) to join our hospice care team in Palo Alto. This role plays a vital part in ensuring patients receive personal, supportive care in the comfort of their home or care facility.

Requirements

• Certified Nursing Assistant (CNA) – Required
• Certified Home Health Aide (CHHA) – Required
• Compassionate, patient-centered approach to care
• Strong communication and interpersonal skills

Nice To Haves

• Previous experience in hospice, palliative care, or home health – Preferred

Responsibilities

• Deliver individualized personal care such as bathing, grooming, and dressing
• Assist patients with daily living activities and comfort measures
• Monitor and report changes in patient condition to the care team
• Provide basic healthcare support such as medication reminders and non-sterile bandage changes
• Offer emotional support and companionship to patients and families
• Educate family members on how to safely support their loved ones
