Certified Home Health Aide Job Description Introduction to the Company Home & Health Care Management is the oldest home health care agency in Northern California. Our agency specializes in delivering comprehensive in-home care and case management for older adults, children, and those living with disabilities across many counties. We use a matrix of Medi-Cal, Medicare and targeted health programs to find solutions that work for our clients. Who You Are: You are a Certified Home Health Aide in California who is interested in working with an interdisciplinary team of health professionals helping individuals that have transitioned from a hospital to home care setting and assisting them to remain in the community You enjoy helping clients achieve their personal goals of remaining in the community. You want to work with a team that inspires professional growth and share our priority of providing quality case management and home health care to the community. Note This is not a position for Caregivers, or Registry Aides. We are accepting applications from Certified Home Health Aides who have completed training as a Home Health Aide and received the necessary license issued from the State of California.

Requirements

  • Patient-interaction skills
  • Experience with ADLs
  • Experience with Hoyer Lifts
  • Ability to multi-task and perform duties in a timely and orderly manner
  • Completed an approved Home Health Aide training course and received Certification as Home Health Aide from the State of California
  • Valid California Driver’s License, and eligible to be insured under our liability policy with a clean DMV report
  • Your own vehicle and proof of current auto insurance
  • Current CPR certification
  • Conditional upon job offer is successfully passing a medical exam that tests amongst other things the ability to move, visual acuity, and hearing acuity.
  • Able to lift to 50 pounds without assistance
  • Sits, stands, bends, lifts, walks, and moves intermittently during working hours

Responsibilities

  • Assists patient with personal care activities including bathing, oral hygiene, preparation and feeding of meals, dressing and undressing, ambulation, and other activities as needed
  • Performs duties as assigned in an accurate and timely manner
  • Notifies the office of any abnormal findings or conditions
