We are seeking professional and compassionate Certified Home Health Aide (CHHA) to provide continuous in‑home support under the supervision of a Registered Nurse and in accordance with an individualized Plan of Care. Requirements
- High school diploma
- Graduate of a state-approved CHHA program
- Access to reliable transportation
- Ability to remain in the client's home for extended live-in shifts
- Current, active certification: Certified Home Health Aid (CHHA) issued by the New Jersey Board of Nursing
- Seeking coverage in multiple counties in South Jersey (Salem, Cumberland, and Cape May)
Responsibilities
- Assist with bathing, grooming, dressing, toileting, and hygiene
- Provide incontinence care and support skin integrity
- Assist with mobility, transfers, ambulation, and positioning
- Perform range‑of‑motion exercises as directed by the supervising RN
- Prepare meals in accordance with dietary needs
- Perform light housekeeping, including laundry, dishes, and general tidying
- Maintain a clean, safe, and organized living environment
- Support household routines and daily structure
- Provide social interaction and emotional support
- Encourage participation in activities, hobbies, and cognitive engagement
- Offer respite support to family caregivers when applicable
- Provide medication reminders (no medication administration)
- Monitor and report changes in condition to the supervising RN
- Record vital signs
- Document daily care activities accurately and promptly
