Union Position: No Department Details Summary Educates patients on self care involved in the diagnosis of diabetes; collects patient data to monitor health status and proactively manage the patient's condition. Job Description Organizes diabetes self care materials including nutrition, medication, physical activity and glucose testing. Educates patients on the necessary lifestyle changes involved in the diagnosis of diabetes. Demonstrates familiarity with pathophysiology, epidemiology, and clinical practice consistent with practice level. Collects patient data on a regular basis, in order to monitor their health status. Provides nutrition therapy and monitors nutritional care of clients/patients of all ages including neonatal, pediatric, adolescent, adult and geriatric. Ability to recognize, evaluates, and provide for nutritional needs based on cultural, ethnic, and religious beliefs. Makes suggestions on how to improve health conditions based on the analysis of patient data; refers patients with specific problems to other healthcare providers. Applies principles of program and/or business management to create a climate that supports successful self-management of diabetes. Educates patients on self-care and lifestyle changes involved in the diagnosis of diabetes.
