About The Position
Duly Health and Care is seeking dedicated team members who are passionate about helping others live happier and healthier lives. The position involves providing diabetes self-management education to patients with various types of diabetes, as well as offering Medical Nutrition Therapy services to a diverse patient population. The role requires a comprehensive assessment of patients' health histories and needs, and the ability to educate patients in both individual and group settings. Team members will collaborate with an interdisciplinary team to create effective care plans and will be expected to maintain professional standards and continuing education requirements.
